Payment & Fees

Transparency in billing is a priority at Acuity. We will provide a detailed estimate of the cost of your services, before your appointment. See our Evaluations page to review our most commonly requested evaluation types and typical fees.

We are out-of network with all insurance plans, except traditional Medicare Part B. This means that you pay us directly on the day of your appointment. We will provide a receipt for you to submit to your insurance to request out-of-network benefits/reimbursement.

Deposit for Evaluations

A credit card must be on file at the time of scheduling.

A non-refundable fee of $250 is charged at the time of scheduling to reserve the date/time of your evaluation.

Final Payment

Final Payment: The fees for clinical services, minus your $250 deposit, is due on the day of your evaluation.

Out-of-network coverage varies and will depend on your specific plan. Check with your insurance carrier before your appointment. Acuity makes no guarantee of coverage.
