Cognitive rehabilitation is an option for clients who have been diagnosed with Mild Cognitive Impairment, Mild Neurocognitive Disorder, or Dementia, or those who have had a brain injury or stroke or other medical condition that can affect thinking abilities. 45-minute, weekly sessions can be conducted individually, or with the client and their support-partner.
The goal of cognitive rehabilitation is to help the client (and family) learn how to build effective routines, organizational, and compensatory strategies, to optimize independence in daily life. Services are customized to client’s cognitive strengths and weaknesses, lifestyle and goals, and based on the underlying condition (e.g., brain injury, Alzheimer’s dementia, Vascular dementia, FTD, aphasia, brain injury, stroke, MS, cancer, etc.).
-
Supportive guidance and education, offered to caregivers, support-partners, and families navigating a new diagnosis or medical condition, day-to-day caregiving challenges, personality changes and behavioral issues in their loved one, and the emotional toll of caregiving. Sessions are designed to help you better understand your loved one’s condition and what the future may hold, set boundaries, navigate medical care/coordination, access community resources (including county-based supports), and take good care of yourself along the way.
